Every Post COVID Care facility offers different services and access to specialists. We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services). *Survivor Corps does not endorse any providers.

Multidisciplinary Long-Term Recovery

Prevea Health is offering a COVID Recovery Clinic at two locations in Wisconsin to help individuals who have had COVID-19 and continue to suffer from associated ongoing post-COVID symptoms long after the initial illness has subsided. The COVID Recovery Clinic brings together multidisciplinary teams and multiple medical specialties in order to coordinate care and help patients thoroughly manage their COVID-19 aftercare.

The Clinic utilizes a comprehensive team approach starting with a clinical assessment by a primary care provider who will work closely with the patient to identify and navigate all phases of their COVID-19 recovery, which may include referrals to other specialists.The Clinic acknowledges and treats common lingering post-COVID symptoms including shortness of breath, cough, fatigue, body aches, joint pain, difficulty sleeping, headaches, brain fog, gastrointestinal issues and dizziness.

Patients who are active adults or youth athletes may also receive return-to-sports assessments. These assessments help determine if individuals are healthy enough to return to the physical demands of their once active lifestyles.

Multidisciplinary Long-Term Recovery

Bellin Health has begun to offer Long COVID Care, a service that cares for patients who continue to experience symptoms such as shortness of breath, cough, fatigue, body aches, joint pain, difficulty sleeping, headaches, brain fog, gastrointestinal issues and dizziness after their initial COVID-19 infection. 

To make an appointment: Call your Bellin Health primary care provider, who will assess your symptoms and, if needed, connect you with additional members of your Bellin care team. If you don’t have a primary care provider, simply call the Long COVID Care Hotline at 920-445-7395 and one of Bellin Health’s trained navigators will get you connected to the care you need.

Multidisciplinary Long-Term Recovery

If you are still experiencing symptoms or ongoing challenges after a bout with COVID-19, including both medical and emotional symptoms, contact the Froedtert & the MCW team. If you are experiencing persistent medical symptoms that negatively impact your daily function eight or more weeks after your COVID-19 diagnosis, please discuss them with your Froedtert & MCW primary care provider to see if our Post-COVID Multispecialty Clinic may be right for you.

To make an appointment: Your primary care physician can refer you to this team of specialists, or you can make an appointment for an evaluation through their system. A physical clinic is expected in summer 2021.

Essentia Health Post COVID-19 Rehabilitation Program

Rehabilitation & Therapy

The Essentia Health Post COVID-19 Rehabilitation Program offers services for patients suffering from long-term effects of COVID-19 illness. The multidisciplinary approach of the Program will look different for each patient and is a combination of physical therapy, occupational therapy and speech therapy.

Therapists use objective and measurable tests to show the progress the patient is making. Patients within this program range from those who were sick with COVID but recovered at home to those who were critically ill and hospitalized. The Program can also refer patients to other providers who may be able to provide additional treatment, such as ear, nose and throat specialists or behavioral health experts.

The Program is offered at all Essentia locations.

Team Rehabilitation Post COVID-19 Recovery Program

Rehab and Physical Therapy

Team Rehabilitation has developed a Post COVID-19 Recovery Program to assist all patients affected by COVID-19 with their recovery and allow them to return to work and daily living. The Program is designed to help patients, using medically supervised therapy, so they may regain strength, respiratory function and resume normal lifestyle.

Throughout the therapy sessions, a patient’s heart rate, oxygen saturation and blood pressure are carefully monitored and that data is used to maintain a safe and effective recovery program.

Evaluation includes assessment of strength, flexibility, respiratory muscle function, and activity tolerance, using a Rate of Perceived Exertion scale.  Standardized functional tests are performed while monitoring vital signs.

The program may include core, upper body and lower body strengthening exercises, a cardiovascular conditioning program, and stretching as indicated. Initially these sessions in the clinic may not be long, based on patient activity tolerance.  The patient’s vital signs are monitored, and exercise is performed within target heart rates, while monitoring oxygen saturation levels.

It is important to string together frequent, short bouts of exercise early on to improve activity tolerance, which means the patient needs to be doing these activities multiple times per day. Patients will be provided with a home exercise program and a pulse oximeter for home use, and instructed how to safely monitor their own response to the exercises. As the patient grows stronger, the intensity  and duration of exercise increases, for both strengthening and endurance.  Supervised exercise time will increase.

If a patient is looking to return to work and is able to perform activities of daily living fairly well, they can be transitioned to a Work Reconditioning Program tailored to their needs, with input from their employer.  At this point they will spend several hours in the clinic performing a variety of work simulation tasks, strengthening exercises, and cardiovascular activities.

The Post COVID-19 Recovery Program can be started in the home via TeleRehab and moved into the clinic at the appropriate time for more advanced activities. A patient’s therapy will be monitored closely in the home setting, via pulse oximeter device, and measured by a Physical Therapist. Our staff is fully qualified to anticipate potential over-stressing situations and will make adjustments to therapy during these monitored sessions to yield the best results for each patient.

The Ascension Medical Group Post-Acute COVID Care team is equipped to provide individualized COVID-19 rehabilitation treatment and support to anyone in the greater Milwaukee area. The Post-Acute COVID Care team will receive any patients who have previously tested positive for COVID-19 and are still experiencing symptoms. The team is prepared to treat symptoms including muscle aches and pains, persistent fatigue, shortness of breath / difficulty breathing, neuropathy, headache, dizziness, anxiety, depression, and other physical and mental symptoms.

The COVID Care team is composed of various experienced specialists such as pulmonologists, cardiologists, behavioral health specialists, neurologists, neuropsychologists, rehabilitation specialists, primary care providers, and others. Additionally, Ascension Medical Group has set up a plasma donation network for any interested patients.

The Courage Kenny Rehabilitation Institute, part of Allina Health, is offering post-COVID care at several locations across Minnesota and western Wisconsin. The Institute is committed to providing holistic rehabilitation services to treat post-COVID patients who have lingering symptoms, whether after being discharged from a hospitalization for acute COVID-19 or exhibiting ongoing symptoms at home. Specialists affiliated with the Institute include physical medicine & rehabilitation providers, physical therapists, occupational therapists, speech therapists, neuropsychologists, pain specialists, and mental health specialists. Care coordination is available to provide support for navigation of treatment and after-care services at the Rehabilitation Institute. Both telehealth and in-person care options are available. Within the Rehabilitation Institute are more specific treatment services, which include:

Post-Acute Inpatient Rehabilitation: Inpatient care is provided by a team of physical medicine and rehabilitation specialists including doctors, nurses, therapists, psychologists and behavioral therapists, social workers, and fitness specialists for those recovering from acute COVID-19 infection. Adult services only.

Post-COVID Rehabilitation Program: Physical medicine & rehabilitation providers, physical therapists, occupational therapists, speech language pathologists, and rehabilitation psychologists are available at this level of treatment to support full recovery from lingering effects of COVID-19. This multi-disciplinary team can help with issues ranging from fatigue, cognitive issues, and balance concerns to breathing difficulty, stress and anxiety. 

Post-COVID Wellness Program: This Program has been designed to establish fitness routines that target common COVID-19 side effects while also improving overall health. No physician referral is needed. Warm water aqua therapy is also available. Virtual options available.
