Multidisciplinary Long-Term Recovery

The University Hospitals COVID Recovery Clinic is dedicated to the comprehensive evaluation, diagnosis and multidisciplinary treatment of patients with Post-Acute Sequelae of SARS-CoV-2 infection (PACS), more commonly referred to as COVID Long-Haul Syndrome. At the UH COVID Recovery Clinic, patients have access to a vast network of medical specialists with the training and experience to recognize the syndrome, perform the appropriate diagnostic tests, and develop a personalized treatment plan to address every physical and behavioral aspect of the patient’s illness.

Located at UH Ahuja Medical Center, the UH COVID Recovery Clinic offers both in-person and virtual care based on the patient’s specific symptoms and needs. The goal of the Clinic is to identify the best way(s) to manage symptoms and coordinate the multidisciplinary care required to restore health.

The UH COVID Recovery Clinic stresses the importance of treating the whole person. Whole-person care places an emphasis on integrative health (stress management, relaxation techniques, acupuncture); behavioral health intervention for depression and anxiety or other psychological issues; rehabilitation services (PT, OT, neuropsychological rehabilitation); general pain management; and pulmonary/cardiac rehabilitation. In addition, patients recovering from COVID often have sleep disturbances, which can lead to general symptoms of fatigue, weakness and cognitive impairment. Therefore, patients at the clinic will also be evaluated for sleep-related health issues and prescribed the appropriate therapies to improve their sleep.

Once you have scheduled an appointment, you will be emailed or mailed a medical questionnaire to complete and send back. This questionnaire will ask you to provide details about your COVID history (dates, symptoms and treatments received) along with a description of your current symptoms and other health issues as compared to your functioning status prior to COVID.

Pediatric Information: Although initial research suggests that fewer children than adults develop COVID Long-Haul Syndrome, there have been reported cases of persistent, often debilitating post-infection symptoms in pediatric patients. These may include diarrhea, extreme fatigue, stomach pain, new onset headache or migraine, and chronic, recurring fever. The pediatric specialists at UH Rainbow Babies & Children’s Hospital are working to create an assessment and treatment model for children that is similar to the adult COVID-19 Recovery Clinic.

Rehabilitation & Therapy

The TriHealth Physical Rehabilitation Outpatient Services (PROS) program offers a comprehensive treatment pathway for those requiring more than one discipline for treatment on an outpatient basis. The PROS team of physical and occupational therapists, speech pathologists, audiologists, psychologists, aquatic therapists and other specialists create a comprehensive, individualized course of therapy to maximize independence.

The program specifically recognizes the tremendous stress that COVID-19 disease and its aftermath can impose on individuals and families, and so offers a specialized course of rehabilitation to speed recovery. The PROs COVID rehab program offers treatment for manifestations that include cardiac, pulmonary, neurologic, muscular and cognitive impairments that can occur.

UC Health Post-COVID-19 Clinic

Multidisciplinary Long-Term Recovery

The UC Health Post-COVID-19 Clinic provides a team of multidisciplinary specialists and subspecialists to help patients who have been diagnosed with long COVID / PASC and have been referred to UC Health by their primary care physicians.

After a patient is diagnosed with possible long COVID / PASC and referred by their primary care provider, an initial consultation is held with a UC Health cardiologist or pulmonologist via telehealth. UC Health specialists will frequently initiate a panel of simple tests to better understand the patient’s condition.The tests will be followed by a face-to-face clinic visit on UC Health’s Clifton Campus with a subspecialist best suited to each patient’s unique condition. The UC Health Post-COVID-19 Clinic includes subspecialists in cardiology, pulmonology, neurology, rheumatology, psychology and dermatology.

In addition to providing a consolidated clinic with the resources to provide cardiac rehabilitation, pulmonary rehabilitation, physical therapy and subspecialty care, the Clinic is also conducting research into long COVID / PASC.

The UC Health Post-COVID-19 Clinic is for patients recovering from significant, lingering conditions of COVID-19 infection. The associated symptoms can be as varied as persistent shortness of breath or difficulty breathing, fatigue, impaired stamina, chest pain, palpitations, dizzy spells, exercise intolerance, wide-swings in blood pressure or impaired concentration. If a patient demonstrates symptoms like this 20 days or more after a COVID-19 infection, his or her primary care physician may consider an evaluation to determine if the patient may need specialized care at the clinic.

Cardiovascular Long-Term Recovery

The Cleveland Clinic Post-COVID Cardiovascular Recovery Program helps patients who have had COVID-19 begin the long-term recovery process. The program uses a multidisciplinary approach to treatment by including Cleveland Clinic doctors and other healthcare providers who are experts in different areas who work together to create a complete plan of care to meet each patient’s specific post-COVID needs. The Program’s multidisciplinary cardiovascular medical team includes specialists from clinical cardiology, cardiovascular imaging, heart failure, electrophysiology, vascular medicine and sports medicine.

Patients in the Post-COVID Cardiovascular Recovery Program will receive:

  1. A complete evaluation to screen for COVID-19 complications that affect your heart and cardiovascular system.

  2. An initial evaluation to screen for other complications affecting your:

    1. Lungs and respiratory system.

    2. Brain and neurological system.

Your appointment will begin by meeting with a nurse or physician assistant (PA) who will collect information about your medical history, symptoms and test results, which will be stored electronically so your doctor can access it during your exam. A cardiologist from the Post-COVID Cardiovascular Recovery Program will review your records, take a medical history and perform a physical exam. You may need blood and imaging tests, such as an electrocardiogram, echocardiogram, MRI or CT scan. Your provider will use these tests to help diagnose your condition and to create an individualized treatment plan for you.

If you need to meet with other multidisciplinary specialists, we will try to arrange a meeting with them within a day to limit your travel as much as possible to and from Cleveland Clinic. The Post-COVID Cardiovascular Recovery Program cardiologist will talk to you about your diagnosis and plan of care within 1 to 2 days, depending on when your test results are completed. You should plan on spending 1 to 2 days in the Cleveland area. Your stay may be longer if treatment is scheduled at the time of your visit.

Your doctor will then talk to you about your plan for follow-up care. Both in-person and virtual visit options are available as appropriate.

The Post-COVID Cardiovascular Recovery Program may be able to help you if:

  1. You are at least 4 weeks past initial COVID-19 infection with any symptoms of heart disease, such as:

    1. Chest pain.

    2. Feeling short of breath.

    3. Feeling dizzy or light-headed.

    4. Passing out or feeling like you might pass out.

    5. Heart palpitations (feel like your heart is beating very fast, skipping beats or fluttering).

    6. Feeling very tired.

    7. Feeling like you can’t do your normal activities.

    8. You were told during your infection that you have a positive heart inflammatory biomarker (NT-Pro-BNP and/or troponin)

  2. You have a history of cardiovascular disease and are worried about how COVID-19 has affected your heart.

  3. You are an athlete and worried about the effects of COVID-19 on your heart and if it will affect your ability to return to play.

  4. You have cardiovascular symptoms related to post-acute sequelae of COVID-19 (PASC)

Cleveland Clinic Post-ICU Recovery Clinic (PIRC)

Rehabilitation and Therapy / Acute COVID-19 Recovery

The intent of the Cleveland Clinic’s Post-ICU Recovery Clinic (PIRC), created by physicians affiliated with the Cleveland Clinic Respiratory Institute is to provide contextualized care for survivors of acute COVID-19 infection resulting in ICU hospitalization. PIRC offers services to any survivor of COVID-19 who has had an ICU length of stay ≥ 72 hours. These survivors are typically able to access care at the Clinic 2-4 weeks after hospitalization. The PIRC offers both telemedicine and virtual medical care.

Patients at the PIRC first receive a pulmonary function test, a six-minute walk distance test, and a chest X-ray. Patients then meet with an interdisciplinary team of healthcare providers, review their ICU medical history, receive a pharmacist’s medication and vaccination review, receive a mental health assessment, and undergo a complete physical evaluation to determine if physical therapy is required. The PIRC is connected to a series of specialty physicians and centers who are equipped to provide personalized care to COVID survivors.

MetroHealth Post-COVID Clinic

Multidisciplinary Long-Term Recovery

The MetroHealth Post-COVID Clinic has been set up to provide virtual clinical appointments for those experiencing long-term health effects as a result of COVID infection. The clinic connects patients who are suffering with lingering symptoms of COVID (such as fatigue, shortness of breath, joint pain, chest pain, anxiety, brain fog, headache, fever, racing heart rate, rash, loss of smell and taste, sleep issues, changes in mood, etc. ) to the appropriate specialist(s) who can determine the best after-care treatment to minimize symptoms through therapy or medication. The Post-COVID Clinic is currently staffed by a rheumatologist, a rheumatology nurse practitioner, a family medicine physician, and an internal medicine physician. Appointments are held via phone or video call. 

  1. Patients can contact (or be referred to) the Post-COVID Clinic if they continue to have symptoms disrupting daily life activities four weeks after testing positive. Those experiencing lingering symptoms less than four weeks after a positive COVID diagnosis should contact their primary care provider if they feel they need medical help.”

The Ohio State University Wexner Medical Center Post-COVID Recovery Program

Multidisciplinary Long-Term Recovery

The Ohio State University Wexner Medical Center  Post-COVID Recovery Program provides comprehensive evaluation and treatment for patients who continue to experience symptoms of COVID-19 longer than four weeks after testing positive for the disease. These lingering symptoms include fatigue, muscle and body aches, shortness of breath, persistent cough, confusion, brain fog and loss of taste and smell.

The program brings together specialists in pulmonology, pulmonary rehabilitation, cardiology, neurology, rheumatology, physical and occupational therapy, sleep medicine and psychiatry to evaluate patients and develop strategies to address the long-term effects and improve their quality of life.

Patients are referred to the Post-COVID Recovery Program by Ohio State Wexner Medical Center physicians. A nurse navigator collects patients’ baseline health measures and medical history before their consultation with an internal medicine physician. A personalized care plan is determined, and patients are promptly connected with specialists who are familiar with the latest recommendations on how to treat long COVID.

The aim of the Memorial Health System Post COVID Care Center is to manage and treat patients who are suffering from long-term effects of COVID-19. The Center is supported by a multidisciplinary team from the following specialties: Physical Medicine, Cardiology, Psychology, Sleep Medicine, and Pulmonary Rehabilitation. The clinic utilizes telehealth to deliver care after your first appointment.The goal of the Center is to provide patients with the correct diagnosis and management options for their post-COVID complications as promptly and efficiently as possible.

Multidisciplinary Long-Term Recovery

The multidisciplinary team of experts that make up the Cleveland Clinic reCOVer Clinic provide help for COVID-19 long-haulers through accurate diagnosis, evidence-based treatment and novel research opportunities. Initial reCOVer Clinic appointments will be in-person or virtual and will last about 60 minutes. During their first appointment, each patient will receive a comprehensive evaluation from clinical experts including taking an extensive medical history and doing a physical exam.

Everyone who is evaluated at the reCOVer Clinic will have:

  1. A chest X-ray.

  2. Pulmonary (lung) function tests.

  3. A 6-minute walk to detect low oxygen.

  4. An electrocardiogram (EKG or ECG) and echocardiogram (echo) to evaluate how your heart functions.

  5. A full physical therapy and occupational therapy evaluation.

  6. Blood work drawn for nutritional, kidney, heart muscle tests.

Following this initial visit, each patient will be scheduled for several tests and lab work and will ultimately be referred to the appropriate specialists to test and treat long COVID symptoms. Patients will have access to specialists from pulmonology, cardiology, neuropsychology, neurology, psychiatry, rheumatology, nephrology, dermatology, ENT, endocrinology, geriatrics, sleep medicine, integrative medicine, functional medicine and others. 

You can be evaluated at the reCOVer Clinic if you’re an established Cleveland Clinic patient who has:

  1. Positive PCR test results for SARS-CoV2 (a positive non-rapid COVID-19 test).

  2. Ongoing or new symptoms at least 28 days after being diagnosed with COVID-19.

To make an appointment: If you'd like to be seen at the reCOVer Clinic, discuss your ongoing symptoms with your Cleveland Clinic provider. They'll put in an electronic order for you to be seen in the COVID reCOVer Clinic. The Clinic team will then reach out to you to schedule an appointment at one of their locations. First appointments are usually scheduled virtually or in-person at Independence Family Health Center. The Clinic is expanding to offer services at several locations. When you are contacted by the scheduling team, they'll arrange your first appointment at a location most convenient for you.


Southwest General COVID-19 Recovery Program

Southwest General offers treatment to help patients recovering from COVID-19 understand their symptoms and work through the limitations that can result from the virus, so they can resume the activities of daily living and return to the life they knew.

The Program offers an experienced team of physical, occupational and speech therapists.  These highly skilled, licensed, experienced therapists are educated to assess, diagnose and treat an injury—or refer you to your physician or a specialist. Experts work one-on-one with each patient to assess his or her level of functioning and recommend a comprehensive, individualized treatment plan to help the patient return to a higher level of functioning.

To participate in the COVID-19 Recovery Program, you must meet guidelines from the Centers for Disease Control (CDC) that define when persons with COVID-19 may discontinue home isolation.
