Multidisciplinary Long-Term Recovery

The Advocate Health Care Post-COVID Recovery Clinic provides a team of expert specialists to work in partnership with a patient’s primary care provider to coordinate the care, testing and treatment plan for those who have recovered from COVID-19 and are experiencing continued symptoms of the virus. The Clinic’s multidisciplinary approach may include (depending on symptoms): pulmonology, physical rehabilitation, neurology, nephrology, hematology, cardiology, behavioral health, and nutrition services. Conditions treated at the Clinic include:

  1. Difficulty breathing or shortness of breath

  2. Tiredness or fatigue

  3. Symptoms that get worse after physical or mental activities

  4. Difficulty thinking or concentrating (sometimes referred to as “brain fog”)

  5. Cough

Jesse Brown’s Long COVID program takes a Whole Health System approach that focuses on individual health and wellness goals to form a treatment plan.  The program works with multiple healthcare specialists, disciplines, and therapies.

Multidisciplinary Long-Term Recovery

At the University of Chicago Medicine Post COVID Recovery Clinic, clinicians will evaluate patients for complications of COVID-19 infection, support them through the recovery process, and connect them with available resources, including specialty care if needed. Patients will have the opportunity to participate in research studies and to enroll in a registry, allowing us to offer treatments should they become available in the future.

Patients do not need to have a test confirmed case of COVID to seek care.

An initial telehealth visit will start with an assessment of the likelihood that a patient had COVID-19. This visit will include a review of symptoms, tests, and previously completed treatments. In addition, they will assess behavioral health needs, quality of life, and impact on work and family.

Multidisciplinary Long-Term Recovery

For individuals with ongoing symptoms of COVID-19, the Northwestern Medicine Comprehensive COVID-19 Center provides access to a team of specialists who can address unique patients needs and guide long-term COVID-19 recovery. The center provides care coordination and can schedule multiple appointments with various specialists if necessary. Patients are evaluated at multiple clinics and by a core group of clinicians who work collaboratively to help streamline care for long-term symptoms of COVID-19.

Through the Comprehensive COVID-19 Center, you will have access to specialists in the following areas, as needed:

Eligibility: The Comprehensive COVID-19 Center welcomes patients who have symptoms related to COVID-19 beyond 30 days from the initial onset of illness or the last positive COVID-19 test.

Specialists in the Comprehensive COVID-19 Center are located in clinics at Northwestern Memorial Hospital, Lake Forest Hospital and Central DuPage Hospital. Appointment locations may vary by provider, and you may need to visit more than one location to receive recommended care. 

You do not need a primary care physician or a referral to seek treatment at this center. However, if you do have a primary care physician, specialists in the center will work with them throughout your treatment to guide your care. If you do not live in the Chicago area, you may be able to access care from certain specialists at the center from anywhere in the United States via phone or video conference.

Multidisciplinary Long-Term Recovery

UI Health's Post-COVID Clinic brings together an expert team of doctors to provide each patient with coordinated, comprehensive care. From neurologists who treat concerns like headaches and concentration problems to pulmonologists who treat concerns like breathing difficulties, the Clinic’s team of doctors work together to help patients on their COVID recovery. Patients will work directly with a care navigator who will help them schedule appointments and follow-up with their team of doctors.

Typically, patients seen in the Post-COVID Clinic:

  1. Have been referred by a primary care physician

  2. Have COVID-related symptoms 30 or more days after having COVID

  3. Had a mild case of COVID but continue to have symptoms

  4. Are experiencing new, chronic conditions like depression, fatigue, weakness, difficulty concentrating, shortness of breath, headaches, kidney problems, and more

Multidisciplinary Long-Term Recovery

The Clay County Hospital Post-COVID Recovery Program is available to address the extended COVID-19 health issues you are experiencing. These may include fatigue, fall or balance problems, weakness and poor endurance, shortness of breath, insomnia, or cognitive impairment.

The Program will coordinate an initial visit with a primary care provider, who will perform a comprehensive examination to identify any complications resulting from COVID-19. After this appointment, an individualized care plan will be prepared based on your symptoms and healthcare needs. If needed, you will be referred to other specialized services to assist in your recovery. These services may include physical therapy, pulmonary rehabilitation, cardiac monitoring, oxygen management, meeting with a dietitian, medication management, and/or a depression screening/cognitive assessment.

Neurological Long-Term Recovery

Patients with long-term (greater than six weeks from initial COVID-19 infection) neurological, cognitive and other neuropsychiatric symptoms associated with COVID-19 can receive multidisciplinary evaluation and care at Loyola Medicine’s Neurology COVID Clinic.

The clinic is led by José Biller, MD, professor and chair, department of neurology, Loyola University Medical Center and Loyola University Chicago Stritch School of Medicine. In addition to Dr. Biller, the Loyola neurology COVID clinic includes neurologists Stasia Rouse, MD and Eugene Schnitzler, MD (for pediatric post-COVID patients).

Edward-Elmhurst Health Post-COVID Neuro Care Clinic

Neurological Long-Term Recovery

Edward-Elmhurst Health has established a Post-COVID Neuro Care Clinic to support the recovery of people experiencing long-term health effects of COVID-19. The goal of the Clinic is to help ease or resolve post-COVID neurological symptoms so that patients can put COVID-19 behind them and move forward with life.

The clinic is staffed by a team of neurology experts at Edward Hospital in Naperville and Elmhurst Hospital. Treatment will vary and may consist of medications, supportive therapies, lifestyle recommendations and referrals to other specialists, including cardiologists, dietitians, gastroenterologists, nephrologists, neuropsychologists, psychiatrists, pulmonologists and others.

When you schedule an appointment at the Post-COVID Neuro Care Clinic, you’ll be asked to complete an online assessment questionnaire prior to your visit. Your responses will help the Clinic better understand your neurologic symptoms and COVID-19 recovery. The Clinic also recommends you keep a diary of your symptoms, including when symptoms occur and how you felt, as well as what makes symptoms better or worse. Bring this information with you to your appointment as well as your current medications, including the dose and frequency.

When you first come to the Clinic, you’ll receive a full evaluation by a multidisciplinary team of neurology experts. The team will:

  1. Spend time talking with you, ask further questions about your COVID-19 diagnosis, and review your symptom diary (if you have one).

  2. Complete a physical and cognitive exam to guide treatment recommendations.

  3. Possibly recommend diagnostic testing to provide more insight into your symptoms, assist with diagnosis and rule out other conditions.

Once the Clinic’s neurology team conducts a full evaluation, they’ll recommend a treatment plan. Your individualized treatment plan may include medications, supportive therapies and lifestyle recommendations. The team may also refer you to other specialists, such as pulmonologists, cardiologists, gastroenterologists, rehabilitation therapists, behavioral health therapists and nutritionists, to ensure you receive comprehensive care for every aspect of your recovery.

Team Rehabilitation Post COVID-19 Recovery Program

Rehab and Physical Therapy

Team Rehabilitation has developed a Post COVID-19 Recovery Program to assist all patients affected by COVID-19 with their recovery and allow them to return to work and daily living. The Program is designed to help patients, using medically supervised therapy, so they may regain strength, respiratory function and resume normal lifestyle.

Throughout the therapy sessions, a patient’s heart rate, oxygen saturation and blood pressure are carefully monitored and that data is used to maintain a safe and effective recovery program.

Evaluation includes assessment of strength, flexibility, respiratory muscle function, and activity tolerance, using a Rate of Perceived Exertion scale.  Standardized functional tests are performed while monitoring vital signs.

The program may include core, upper body and lower body strengthening exercises, a cardiovascular conditioning program, and stretching as indicated. Initially these sessions in the clinic may not be long, based on patient activity tolerance.  The patient’s vital signs are monitored, and exercise is performed within target heart rates, while monitoring oxygen saturation levels.

It is important to string together frequent, short bouts of exercise early on to improve activity tolerance, which means the patient needs to be doing these activities multiple times per day. Patients will be provided with a home exercise program and a pulse oximeter for home use, and instructed how to safely monitor their own response to the exercises. As the patient grows stronger, the intensity  and duration of exercise increases, for both strengthening and endurance.  Supervised exercise time will increase.

If a patient is looking to return to work and is able to perform activities of daily living fairly well, they can be transitioned to a Work Reconditioning Program tailored to their needs, with input from their employer.  At this point they will spend several hours in the clinic performing a variety of work simulation tasks, strengthening exercises, and cardiovascular activities.

The Post COVID-19 Recovery Program can be started in the home via TeleRehab and moved into the clinic at the appropriate time for more advanced activities. A patient’s therapy will be monitored closely in the home setting, via pulse oximeter device, and measured by a Physical Therapist. Our staff is fully qualified to anticipate potential over-stressing situations and will make adjustments to therapy during these monitored sessions to yield the best results for each patient.

SIU Medicine has created a Post-COVID Recovery Clinic to support COVID-19 long-haulers. Patients can receive customized care from a team of multidisciplinary specialists. Clinicians will help identify achievable health goals as a way to optimize patients’ quality of life and function. The Recovery Clinic will emphasize shared decision-making between providers and patients, and will be tailored to address a patient’s individual long-term COVID recovery needs.
