Every Post COVID Care facility offers different services and access to specialists. We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services).

Note: At this time, Survivor Corps has only found clinics on the Hawaiian islands of O’ahu and Kaua’i. We will continue to search for post-COVID resources for long-haulers across Hawai’i.

Multidisciplinary Long-Term Recovery
The Queen’s Health Systems (QHS) COVID-19 Community Ambulatory Recovery and Evaluation (CARE) Program provides specialized after-care to patients diagnosed with COVID-19 and are in recovery. As part of the program, the Post-COVID Recovery and Care Clinic provides QHS and community patients with diagnostic services and complete cardiovascular, pulmonary, and neuropsychiatric evaluation.
Eligibility to enroll in the Post COVID Recovery and Care Clinic:
Patient must have a confirmed positive COVID-19 infection diagnosis
Patient must be at least 10 days’ post COVID infection with no current signs or symptoms of active infection (fever ≥100.4ºF, chills, cough, shortness of breath or difficulty breathing, etc.)
Patient must be referred by their Primary Care Physician using the Referral Form

Hawaii Pacific Health Long COVID Care Program (LCCP)
Multidisciplinary Long-Term Recovery
COVID-19 can cause lasting multi-organ symptoms. The Hawaii Pacific Health Long COVID Care Program (LCCP) supports patients experiencing symptoms of Long COVID. Clinicians connect with patients through telephone and video visits to provide regular monitoring and coordinate care with patients’ primary care providers to manage symptoms and provide referrals to specialists for ongoing care if needed. Specialists may include neurologists, pulmonologists, psychologists, cardiologists, infectious disease, pediatrics, social work and more. Please talk with your primary care provider to see if the LCCP could benefit you.
