Every Post COVID Care facility offers different services and access to specialists. We have labeled each according to services offered: Multidisciplinary Long-Term Recovery (most comprehensive), Rehabilitation & Therapy (focus on rehabilitation) and Acute COVID-19 Recovery (focus on improved function after severe illness or hospitalization, often in-patient services). *Survivor Corps does not endorse any providers.

Rehabilitation & Therapy

The Other Pain Clinic Inc COVID Rehab & Survivorship Program is committed to treating people with post COVID symptoms to allow them to have a better quality of life. All patients enrolled in the Program will participate in an initial consultation appointment that will focus on medical history and current long COVID symptoms.

Medical documentation will occur before, during and after the program using established health outcome measures such as the Post COVID Syndrome Questionnaire, the Post COVID Functional Assessment, the Yorkshire Rehabilitation Scale, and the EQ5 Quality of Life Survey. The Program offers three different treatment pathways depending on the degree to which treatment is tailored to the patient’s individual needs.

The treatment pathways are as follows: 

  1. The self-directed e-book/guide program for the At-Home COVID Health Kit - In this program there will be an initial clinical session, 2-3 follow-up sessions for any Q & A, and then re-evaluation with the health outcome measures. This is typically a 30 day program. 

  2. The 30 Day Coaching Program will be similar to the self-directed At-Home COVID Health Kit program, but will include a weekly session with CEO & Chief Medical Exercise Specialist Francois Gagnon to focus on weekly action-oriented objectives/goals based on the patient’s symptoms. The program typically features health lifestyle behaviors that help the person holistically.  

  3. The 1-on-1 Rehab Program - this is tailored and more in depth than the previous two programs, as it is more specific to what the patient's primary concerns are.

Multidisciplinary Long-Term Recovery

The Edmonton North PCN has brought together a number of primary care physicians, clinicians and specialists to help treat COVID-19 symptoms that you are still experiencing 10 weeks after receiving your positive test result, or if you are still experiencing long haul symptoms after a presumptive/suspected case of COVID-19. Some examples of persistent symptoms include but are not limited to:

  1. Fatigue

  2. Shortness of breath

  3. Memory problems

  4. Mental health concerns

  5. Persistent cough

  6. Muscle soreness

If you are referred to the PCN COVID-19 Recovery Clinic, your first appointment will be with a nurse to go over the symptoms you are experiencing so the Clinic can provide you with the right support. Support may be provided from the following specialities:

  2. Exercise Therapy

  3. Mental Health Support

  4. Neurology

  5. Nursing

  6. Physiotherapy

  7. Pulmonary

  8. Rheumatology

  9. Social Work Support

Any patient who is still experiencing COVID-19 symptoms 10 weeks or longer after a positive test or had a presumptive case of COVID-19 is able to receive treatment through the PCN COVID-19 Recovery Clinic.

Lifemark Health Group Post COVID-19 Rehabilitation and Recovery Program
(Multiple locations across six provinces)

The Post COVID-19 Rehabilitation and Recovery Program has been formed by Lifemark Health Group to provide virtual and in-person treatment services to COVID survivors across Canada who have recovered from initial infection but are still struggling with lingering physical and mental complications of the virus. The program emphasizes a multidisciplinary approach to treatment that includes physicians from 15 disciplines, including: Physiotherapy, Occupational Therapy, Vestibular Therapy, Kinesiology, Athletic Therapy, Chiropractic, Massage Therapy, Psychology / Social Work, Dietician, and Speech Language Pathology.

The Program will tailor its rehabilitation services to meet each patient’s individual needs, starting with an initial appointment with a clinician to discuss medical history and holistic health assessment.

Multidisciplinary Long-Term Recovery

The Fraser Health Post COVID-19 Recovery Clinic is a service for patients in Fraser Health who have had COVID-19 and are experiencing complex chronic health issues related to COVID-19 at least 3 months post acute infection. The Clinic runs 8:30 a.m. – 4:30 p.m. from Monday to Friday.

Providence Health Care: Post COVID-19 Recovery Clinics

Providence Health Care, in collaboration with Vancouver Coastal Health, Fraser Health Authority and others, has developed new clinics at St. Paul’s Hospital and at Vancouver General Hospital to provide specialized care and follow-up for patients recovering from COVID-19.

The Clinics provide access to care, services, and education for people who have previously been diagnosed with COVID-19 and are still in the process of rehabilitation, incorporating a combination of on-site and telehealth-enabled clinics that allow for holistic engagement with patients, their GP’s, our allied health team, and our network of subspecialists.

Montréal Clinical Research Institute (ICRM) Post-COVID-19 Clinic

Multidisciplinary Long-Term Recovery

The Montréal Clinical Research Institute (ICRM) Post-COVID-19 Clinic provides informed post-COVID patient care and connects patients to research trials to help doctors evaluate, treat and better understand the full impact of the disease. The ICRM’s Clinic is staffed by a team of specialists for each organ system, dietitians, and kinesiologists. The clinic will closely evaluate each patient’s lifestyle and habits in order to provide the most contextualized support for managing and treating symptoms.

Patients interested in treatment through the Post-COVID-19 Clinic will first be screened online, using questionnaires to gather information about their dietary history, their general well-being and activity level. An in-person appointment will then be scheduled and will include a complete physical exam. Admitted patients will be asked to give blood, urine and stool samples. The timeline for treatment at the Clinic is not yet established, but the goal of the Clinic is to follow patients for five years in order to collect robust research on the experiences of post-COVID individuals. 

Individuals interested in receiving treatment through the ICRM Post-COVID-19 Clinic must reside in Québec, be 18 years of age or older, and have documentation of a COVID-positive PCR or antibody test.

CIUSSS West-Central Montreal Referral Clinic for Long COVID and Lyme Disease

Multidisciplinary Long-Term Recovery

CIUSSS West-Central Montreal has opened a clinic for patients with persistent symptoms of COVID-19 and Lyme disease. 

The clinic offers multidisciplinary care to patients with persistent symptoms of COVID-19 or Lyme disease. The complexity of these two diseases requires the expertise of those in several specialties. Therefore, medical and/or rehabilitation services based on each patient’s individual needs are provided by nurses, specialized physicians and allied health professionals (physiotherapy, social work) in a face-to-face or virtual setting. The specialized clinic allows for an integrated multidisciplinary follow-up to treat patients who have this new pathology or who have Lyme disease.

The clinic is located at the Jewish General Hospital, Pavilion H (5790 Côte-des-Neiges), room H-132.

Eligibility: To be eligible to receive long COVID treatment through this clinic, you must satisfy all of the following conditions:

    1. Be at least 18 years old

    2. Have a positive COVID-19 test (PCR or rapid test) or have been diagnosed with COVID-19 by a healthcare provider

    3. Have new COVID-19 symptoms that were not present before the infection, and that have persisted for at least 12 weeks

    4. Be a resident of Quebec or a student residing in Quebec. Priority will be given to residents of the area served by CIUSSS West-Central Montreal and the Montreal region, as indicated in an agreement with the other designated centres.
